Once your child has been scheduled, a completed intake form must be submitted before their initial appointment. We bill insurance companies as a courtesy, however any co-payment or denial of claims will be the family’s responsibility. For insurance that we are not in-network with, we will provide a monthly super bill for families to seek insurance reimbursement.
